2. High School Transcript w/most recent cumulative GPA
3. Class ranking or national test scores
4. Letter of recommendation from high school principal or counselor
5. Letter of recommendation from PHBA National Director or Youth Advisor
6. Letter of recommendation from faculty advisor or member (for applicants already in college)
7. Recent photograph
8. Income tax return of guardian
9. Scholarship Application
10. Letter from applicant expressing why they wish to obtain a scholarship
